Currently we use the Essex planning tool which is available from the pshe association for around £15. It splits objectives into key themes and is organised in a progressive spiral from Reception to Year 6.
It is very easy to follow and comes with a range of resources. I came across this on a PSHE course years ago and have been reassured that it is still relevant and has great coverage.
I used Health for life (which you can find on amazon) on its own prior to that. It has loads of great lessons and you could follow it through page by page. The problem is there are no explicit LTP and MTP. You have to trust your staff to build their units based on the children's prior knowledge. (which would be ideal!!)
I know use Health for life and Real Health for Real Lives in conjunction with the Essex planning tool. The tool refers to pages and activities in both sets of books and you can use them to their real potential.
